By completing your enrollment, you are agreeing to these terms and will be held to the terms and conditions in this form.

Enrollment 

Enrollment is on a first-come, first served basis.  Full payment is required at time of registration to reserve a space in class/playgroup(s). Please read our cancellation and refund policies before enrolling.                                                                                                                                    

 Training Methods

Our progressive, force-free training programs are based on the scientific principles of animal learning theory and emphasize the use of positive reinforcement based techniques. Physical punishment is not necessary and will not be allowed in the classroom.

 Vaccinations/Health Requirements/Risks

Please make sure your puppy has been examined by your veterinarian for general health, is free of external and internal parasites, and has had the first vaccinations as recommended by your veterinarian.  It is your responsibility to monitor your puppy\u2019s health daily.  If your puppy seems ill or does not eat or play as usual, please take puppy to your veterinarian immediately.  Seattle Puppyworks, LLC requires puppies to have had at least one round of shots, adult dogs must be fully vaccinated. By enrolling your dog in class, you are confirming that your dog/puppy is adequately vaccinated.

Seattle Puppyworks, LLC is aware of varying and developing opinions regarding canine vaccination. Because of this, we encourage puppy owners to to receive recent puppy vaccination position statements from two top veterinary schools in the US (with crucial information contrary to what most breeders and some vets still believe about puppies requiring three full sets of shots before being around other dogs).

 Disruptive/Aggressive Behavior

In order to protect all participants in a class, we reserve the right to decline entry to any dog who presents behavior issues that may be overly dangerous, disruptive, or that may be exacerbated by participation in a group class or playgroup.  If behavior issues are discovered during the class or playgroup and the trainer determines that it is in everyone's best interest that your dog be removed from class, you will be provided the prorated amount of the class series at a cost of $25/class or remainder of play club punch card at purchase value.

                                                                                                                                            

Cancellations

If the minimum enrollment for a class and/or playgroup is not met 1 - 4 days prior to the session, the session will be cancelled. We will notify you at least 24 hours before the session was scheduled to begin to see if you would like to enroll in an alternative class and/or playgroup (if possible), receive credit toward a future class and/or playgroup, or receive a full refund.                                                                                                                                       

 

Refunds

Refunds can be provided if we are notified prior to the first day of a class.  Missed classes can be made up on alternate days, if possible, with instructor permission only.  Missing more than two classes will be considered dropping out.  There are no refunds for missed classes.  There is a $25 fee for all returned checks. If you've paid with a credit card and are eligible for a refund, a $25 processing fee will be deducted from the amount refunded. PLEASE NOTE THAT THERE IS A 2.9% ADMINISTRATIVE FEE FOR ALL CREDIT CARD REFUNDS INITIATED BY THE CUSTOMER. CASH OR CHECK PAYMENTS ARE NOT SUBJECT TO THIS FEE.

 Because Seattle Puppyworks classes are front-loaded with material, refunds are not given once you have attended a class.  Please note that confirmation is sent via email prior to the first day of class. Non-receipt of this confirmation is not considered acceptable reasoning for non-attendance.                                                                                                                             

 Attendance

If you miss a class (other than orientation) please contact the trainer to discuss what was covered. Our classes are taught as a series; therefore, drop-ins and make-up classes are not permitted. Students who were not pre-enrolled in a class can join at the trainer's discretion, provided there is space available in the class.

 Children

Children under the age of 10 are not permitted without instructor permission, and no children under 18 are allowed without an adult guardian. If you have a young child you'd like to bring to class or play, please make arrangements with the instructor BEFORE you complete your registration. 

Puppy Play Club: 

All play sessions must be used before puppy is 18 weeks old. Some exceptions may be made at discretion of Seattle Puppyworks\u2019 staff, prior approval is required.  Purchased but unused play sessions are not eligible for refund unless none of the sessions were used and membership was \u201Cactivated.\u201D Free puppy play cards may not be redeemed for cash value.  Seattle Puppyworks staff is in charge of playgroups and will direct all activity. Owners exhibiting behavior unfit for the playgroup will be excused by staff and a refund will be issued for remaining play sessions at purchase price.  Most puppies need only 20-30 minutes of play per session. more play time than necessary can be detrimental to proper puppy development. Staff will determine appropriate play times for puppies and decisions to end daily play for each puppy will be at their sole discretion.  Play is directed by the highly skilled SPW staff. We ask that all attendees leave play direction to staff members avoiding any punishment or handling of others\u2019 puppies.  Expiration dates for puppy play club memberships are based on puppy age: all puppy play club memberships expire when a puppy is 20 weeks (5 mos) old. 

Waiver and Release

I understand that participating in Seattle Puppyworks classes and/or playgroups involves risks of personal injury and/or property damage to myself, to my dog, to members of my family and to accompanying friends.  I assume all risks associated with participating in classes and/or playgroups.  I agree to comply with the instructions, rules and decisions of the Instructor(s) as it relates to myself or to my dog\u2019s ability to safely complete this class and/or playgroup(s).  I certify that my dog is current on all vaccinations.  In consideration of, and as inducement to the acceptance of my application for classes and/or playgroups, I hereby agree to indemnify and hold harmless Seattle Puppyworks, LLC, Richard Major and their officers, directors, instructors, agents, employees and/or representatives harmless of any and all claims, or claims by any member of my family, or accompanying guests of mine, injury, expense, costs or damages to myself, my dog or any handler sponsored by me, arising out of or in connection with or in any way related to classes and/or playgroups, even though the liability may arise out of negligence or carelessness on the part of the individuals or entities released in this Waiver and Release.  In addition, I agree that I will defend and indemnify Seattle Puppyworks, LLC, Richard Major and their officers, directors, instructors, agents, employees and/or representatives for any injury, expense, costs or damages to any dog handlers or dogs, whether sponsored by me or not, or to third parties arising out of my own actions or the actions of my dog. I agree that the risks of enrolling my dog or puppy in this class have been fully explained to me, and I have been advised to consult with my veterinarian regarding the health risks involved. I also understand that Seattle Puppyworks, LLC may use for publicity or promotional purposes, my name and pictures of me or of my dog without liability or obligation to me.  I have read, understand and agree to the above.
